This is what you send to the Court in order to issue a Garnishment on a case that has already been filed with the Court. 

Important Document Information

Parties ["entity()"s] to be sent: One person, an Obligor or an Obligee, one Attorney and one Garnishee.  If you don't send a an Obligor or an Obligee or send more than one, an error will occur and the Garnishment entry will be rejected.  If you don't send a garnihsee or send more than one, an error will occur and the Garnishment entry will be rejected.    If you don't send an attorney or send more than one, an error will occur and the Garnishment entry will be rejected.  Other parties will be ignored.
