This is what you send to the Court in order to issue an Affidavit of Military Service on a case that has already been filed with the Court.

Important Document Information

Parties ["entity()"s] to be sent: One Defendant (the one for whom the Affidavit applies) and one Attorney for the Plaintiff (the filing attorney).  If you don't send a defendant or send more than one, an error will occur and the Affidavit entry will be rejected.  If you don't send an attorney for the plaintiff or send more than one, an error will occur and the Affidavit entry will be rejected.
