This is what you send to the Court in order to issue a Garnishment on a case that has already been filed with the Court. 

Important Document Information

Parties ["entity()"s] to be sent: One Defendant, one Attorney for the Plaintiff (the filing attorney) and one Garnishee. If you don't send a defendant or send more than one, an error will occur and the Garnishment entry will be rejected. If you don't send a garnishee or send more than one, an error will occur and the Garnishment entry will be rejected. If you don't send an attorney for the plaintiff or send more than one, an error will occur and the Garnishment entry will be rejected.  Other parties will be ignored.
