This is what you send to the Court in order to issue an Alias Summons on a case that has already been filed with the Court. 

Important Document Information

Parties ["entity()"s] to be sent: One Defendant (the one to whom the Alias Summons will be issued) and one Attorney for the Plaintiff (the filing attorney).  If you don't send a defendant or send more than one, an error will occur and the Alias Summons entry will be rejected.  If you don't send an attorney for the plaintiff or send more than one, an error will occur and the Alias Summons entry will be rejected.  Other parties will be ignored.
